As Saaniya (Kubra Khan) hangs between family and hospital duties, her ethical principles are constantly tested as she struggles to stand up to her father-in-law and expose his corruption. The love for her husband and his duty as a doctor stretches her loyalty and honour simultaneously.

Salman (Shuja Asad), weighed down by guilt, is supporting his ex-girlfriend, Faryal (Erica Robin), as she searches for a job in a new city while caring for their child. He can be seen with her, driving her around and helping her find work, as she still hopes for a place in his life. Regardless of his love for his wife, the presence of another woman has become a bone of contention between them.

In his explanations, the trust he expects is not reciprocated regarding Saaniya and her fight against crime in the hospital. His anger and defensiveness toward his family, even though Saaniya has proof against them, are what stand in the way. On one hand, he is supporting his ex-girlfriend, on the other, he is defending his family pointlessly; he has left Saaniya insecure as a wife and unprotected as a professional. While she wishes to preserve her family’s happiness, her sister’s wedding, and her place in the Shahnawaz (Shezad Nawaz), she also wants to save her marriage, which hangs by a thread.

A woman who remains logical in tiring situations is forced to choose motherhood to save her marriage, urging her husband to take a step forward in their marriage and plan a child. Though she is not ready for this step in life, she is forced to believe she can hold on to Salman if she bears his child. It is unlike Saaniya, yet she is only a vulnerable woman, trying to salvage her damaged relationship with her husband, with an ex-girlfriend lingering in the background.

As Saaniya acts unlike herself, netizens are divided as she urges Salman to move forward and have a child; criticising how a woman who stands up against all evils is falling for a conventional solution towards her issues. Calling it a classic attempt to put down a powerful female lead, a user posted on X and said:

Knowing how educated Saaniya is, hr stance to have a child did not sit well with another user and they posted:

However, in her defence, a user stated that Saaniya is a woman in love and feels insecure around another woman, which makes her stance natural. They posted:

While Saaniya is motivated by emotion, Salman’s logical response to her plea is what sets Doctor Bahu apart from classic tropes. He not only makes sure his wife knows his side of the story, but also rejects her request, stating that this decision should not be made out of fear but out of love. Appreciating his approach, a user posted on X and said:
